英国药品和医疗产品管理局（MHRA）2015年9月发布新的一期药品安全性更新月报，介绍了质子泵抑制剂（PPI）和亚急性皮肤型红斑狼疮（SCLE）病例发生的相关性。

PPI可减少胃酸分泌，是广泛用于治疗胃酸相关疾病的药物，包括：反流性食道炎、胃十二指肠溃疡和Zollinger-Ellison综合征。英国市场上销售的PPI包括：埃索美拉唑、兰索拉唑、奥美拉唑、泮托拉唑和雷贝拉唑。

SCLE是一种发生于日光暴露区域的无瘢痕形成的皮肤病。SCLE的特征为多环形红斑鳞片状斑块或融合的银屑病样斑丘疹病变，有时伴有关节痛。可使用皮肤试验（如直接免疫荧光）和血清学试验（包括出现抗Ro抗体或Sj？gren 综合征相关抗原A[SSA]）来诊断SCLE。药物诱导的SCLE可发生于药物暴露后数周、数月，甚至数年。

相关性证据

PPI的应用非常广泛，报告的SCLE病例则相对罕见。尽管如此，临床文献和报告至药品监管部门的病例（包括黄卡计划）的证据仍支持PPI与SCLE之间存在因果关联性。目前正在更新产品信息，以纳入此新的信息。

来自瑞典的一个链接患者注册与处方药物注册系统的病例对照研究认为，接受PPI治疗的患者发生SCLE的风险为普通人群的3倍（OR值为2.9[95% CI: 2.0–4.0]）¹。

丹麦一家大学教学医院中的一个皮肤病科室19年间的患者病历审查中发现了19例与PPI相关的SCLE病例。其中，3例病例被明确归类为PPI所致，14例病例归类为很可能为PPI所致²。

临床文献中还报道了另外17例PPI使用后的SCLE病例^3-8。

截至目前，文献审查的病例和PPI上市许可证持有者向药品监管部门提交的病例报告中，共发现了36例去激发阳性和4例再激发阳性的病例报告。
